Can be with acute onset and marked symptoms in the form of acute chest pain. Generally with basilar infiltrates and blunting of costophrenic angles. Discrepancy between the acute chest symptoms and minimal changes on imaging. Tends to relapse if patient rechallenged with the drug. At times called 'acute pleuro-pulmonary reaction'
