De novo or superipmosed on a background of preexisting PAH. Range from mild to catastrophic and fatal. May result from acute pulmonary vasoconstriction, extensive microembolism, or rebound vasoconstriction upon brisk withdrawal of pulmonary vasodilator therapy.
